The evaluation of bacteriology in perianal abscesses of 81 adult patients BJID
Ulug,Mehmet; Gedik,Ercan; Girgin,Sadullah; Celen,Mustafa Kemal; Ayaz,Celal.
OBJECTIVE: Treatment of perianal abscesses requires prompt surgical drainage and antimicrobial therapy. However, we should encourage the selective use of antimicrobial agents on a case-by-case basis, especially because there is no evidence that uncomplicated perianal abscesses can be safely treated only with drainage. For this reason, it is important to identify the causative organisms; therefore, we accessed the microbiological analysis of these patients. PATIENTS AND METHODS: In this study, 81 consecutive adult patients with perianal abscesses, who presented at a university hospital in Diyarbakir from January 2004 to December 2006, were included. Clinical and laboratory data, and results of microbiological analysis were recorded. RESULTS: All specimens,...

The impact of ocular tuberculosis on vision after two months of intensive therapy BJID
Oliveira,Suzana Batista Vereza de; Passos,Ângelo Ferreira; Hadad,David Jamil; Zbyszynski,Lorena; Sousa de Almeida Júnior,Pedro; Castellani,Luiz Guilherme Schmidt; Dietze,Reynaldo; Palaci,Moisés.
ABSTRACT Tuberculosis is an infectious disease of global importance with major economic and social burden accounting for 25% of all avoidable deaths in developing countries. Extrapulmonary involvement may occur either in association with clinically apparent pulmonary tuberculosis or in isolation. This cross-sectional descriptive study aimed to evaluate the impact of ocular tuberculosis in visual acuity at baseline and after two months of intensive anti-tuberculous therapy. A sample of 133 pulmonary tuberculosis patients, seven disseminated tuberculosis, and three pleural tuberculosis patients was evaluated. All patients underwent routine ophthalmic evaluation, including assessment of visual acuity, biomicroscopy, applanation tonometry, indirect...

The Mathematical Biology of Human Infections Ecology and Society
Nowak, Martin A; Institute for Advanced Study; nowak@ias.edu.
Humans are constant victims of infectious diseases. Biomedical research during this century has led to important insights into the molecular details of immune defense. Yet, many questions relating to disease require a quantitative understanding of the complex systems that arise from the nonlinear interactions between populations of immune cells and infectious agents. Exploration of such questions has lead to a newly emerging field of mathematical biology describing the spread of infectious agents both within and between infected individuals. This essay will discuss simple and complex models of evolution, and the propagation of virus and prion infections. Such models provide new perspectives for our understanding of infectious disease and provide guidelines...

The measurement of urine pH to predict the amount of buffer used in the treatment of acute rumen lactic acidosis in cattle Ciência Rural
Maruta,Celso Akio; Leal,Marta Lizandra do Rego; Mendes Netto,Daniel; Mori,Clara Satsuki; Antonelli,Alexandre Coutinho; Ortolani,Enrico Lippi.
The purpose of the present study was to establish a practical, fast, precise and low-cost procedure to estimate the degree of metabolic acidosis in cattle with acute rumen lactic acidosis for further treatment. The rumen acidosis was induced experimentally in 40 crossbreed rumen-cannulated 1.5-year-old steers. The induction caused the development of the most characteristic clinical signs of acute rumen lactic acidosis, severe rumen acidosis and a moderate metabolic acidosis, which was evidenced by low blood pH, and blood bicarbonate concentration and base excess (BE). A highly positive correlation (r=0.80) between urinary pH and BE concentration, and between urinary pH and blood pH (r=0.75) was observed. The BE concentration estimated by urinary pH was...

The microbiome and inborn errors of metabolism: Why we should look carefully at their interplay? Genet. Mol. Biol.
Colonetti,Karina; Roesch,Luiz Fernando; Schwartz,Ida Vanessa Doederlein.
Abstract Research into the influence of the microbiome on the human body has been shedding new light on diseases long known to be multifactorial, such as obesity, mood disorders, autism, and inflammatory bowel disease. Although inborn errors of metabolism (IEMs) are monogenic diseases, genotype alone is not enough to explain the wide phenotypic variability observed in patients with these conditions. Genetics and diet exert a strong influence on the microbiome, and diet is used (alone or as an adjuvant) in the treatment of many IEMs. This review will describe how the effects of the microbiome on the host can interfere with IEM phenotypes through interactions with organs such as the liver and brain, two of the structures most commonly affected by IEMs. The...

Traitement des crevettes contre le noircissement .Mise au point d'une ligne de traitement ArchiMer
Chantreau, Patrick.
Once fished, the shrimps must be cooled quickly in water cooled in order to slow down all chemical and bacteriological activities. In addition, in order to avoid blackening (mélanose) shrimps (céphalothorax and legs) it is necessary to treat those with an antioxydant product. The product currently used to inhibit this enzymatic change is the soda niétabisuifite (Na-> S7 O5), until now, after being fished. the shrimps were directly soaked in a cooled water tank, containing the pesticide. The solution of treatment was progressively prepared needs. The concentration of this one was approximately 2 to 3% of métabisulfite, that is to say 6 to 9 G of SO2 per liter for a starting product containing 320 G of SO2 approximately. In order to preserve a good...

Traitement des langoustines et des crevettes contre le noircissement ArchiMer
Chantreau, Patrick; Vallet, Jean-luc.
The conservation of crawfish requires the use of chemical substances making it possible to limit blackening. This phenomenon this blackening (melanine) is due to the coordinated action of three elements: - oxygen, - an enzyme (cupro-protein), - a suitable substrate, tyrosin or the dihydroxyphénylalanine. The absence of the one of them allows a notorious reduction in the phenomenon. Several antioxydant products, were used to prevent the formation of the mélanine. The métabisutfite of soda or potassium as well as soda bisulfite in acid solution, are currently used to treat crawfish and shrimps. Several methods of use of these pesticides exist, we propose in this report to present the principal ones, as well as the control methods making it possible to...

Treatment of a subdural empyema complicated by intracerebral abscess due to Brucella infection BJMBR
Zhang,J.; Chen,Z.; Xie,L.; Zhao,C.; Zhao,H.; Fu,C.; Chen,G.; Hao,Z.; Wang,L.; Li,W..
A 55-year-old male presented with fever, stupor, aphasia, and left hemiparesis. A history of head trauma 3 months before was also reported. Cranial magnetic resonance imaging revealed slight contrast enhancement of lesions under the right frontal skull plate and right frontal lobe. Because of deterioration in nutritional status and intracranial hypertension, the patient was prepared for burr hole surgery. A subdural empyema (SDE) recurred after simple drainage. After detection of Brucella species in SDE, craniotomy combined with antibiotic treatment was undertaken. The patient received antibiotic therapy for 6 months (two doses of 2 g ceftriaxone, two doses of 100 mg doxycycline, and 700 mg rifapentine for 6 months) that resulted in complete cure of the...

Treatment of methicillin-resistant Staphylococcus aureus in Latin America BJID
Luna,Carlos M; Rodríguez-Noriega,Eduardo; Bavestrello,Luis; Gotuzzo,Eduardo.
The global spread of methicillin-resistant Staphylococcus aureus (MRSA) means it is now a pathogen of worldwide public health concern. Within Latin America, MRSA is highly prevalent, with the proportion of S. aureus isolates that are methicillin-resistant on the rise, yet resources for managing the infection are limited. While several guidelines exist for the treatment of MRSA infections, many are written for the North American or European setting and need adaptation for use in Latin America. In this article, we aim to emphasize the importance of appropriate treatment of MRSA in the healthcare and community settings of Latin America. We present a summary of the available guidelines and antibiotics, and discuss particular considerations for clinicians...

Trichomoniasis in Bothrops jararaca (serpentes, viperidae) J. Venom. Anim. Toxins incl. Trop. Dis.
Vilela,F. C.; Silva,M. G. da; Barrella,T. H.; Silva,R. J. da.
We describe a case of trichomoniasis in a Bothrops jararaca (Serpentes, Viperidae) donated to the Center for the Study of Venoms and Venomous Animals - CEVAP/UNESP. The animal had diarrhea with great quantity of flagellated protozoa in the feces. Microscopic examination of fecal smears stained with Giemsa revealed the presence of trichomonads, morphologically similar to Trichomonas acosta. Trichomonads were not detected in fecal exams after treatment with a single dose of 40 mg/kg metronidazole (Flagyl®).

Use of antivenoms for the treatment of envenomation by Elapidae snakes in Guinea, Sub-Saharan Africa J. Venom. Anim. Toxins incl. Trop. Dis.
Baldé,Mamadou C; Chippaux,Jean-Philippe; Boiro,Mamadou Y; Stock,Roberto P; Massougbodji,Achille.
Background In Guinea Elapids are responsible for 20% of envenomations. The associated case fatality rate (CFR) ranged 15-27%, irrespective of treatment. Results We studied 77 neurotoxic envenomations divided in 3 groups: a set of patients that received only traditional or symptomatic treatments, and two other groups that received either 2 or 4 initial vials of Antivipmyn® Africa renewed as necessary. CFR was 27.3%, 15.4% and 17.6%, respectively. Although antivenom treatment was likely to reduce CFR, it didn’t seem to have an obvious clinical benefit for the patients, suggesting a low treatment efficacy. Mean delay to treatment or clinical stages were not significantly different between the patients who recovered and the patients who died, or between...

Use of rhu-GM-CSF in pulmonary tuberculosis patients: results of a randomized clinical trial BJID
Pedral-Sampaio,Diana Brasil; Netto,Eduardo Martins; Brites,Carlos; Bandeira,Antonio Carlos; Guerra,Conceição; Barberin,Maria Goreth; Badaró,Roberto.
It has been postulated that deficient or incomplete clinical and/or microbiological response to tuberculosis treatment is associated with cell-mediated immunological dysfunction involving monocytes and macrophages. A phase 2 safety trial was conducted by treating patients with either recombinant human granulocyte-macrophage colony-stimulating factor (rhu-GM-CSF) or a placebo, both in combination with anti-tuberculosis chemotherapy. Thirty-one patients with documented pulmonary tuberculosis were treated with rifampin/isoniazid for six months, plus pyrazinamide for the first two months. At the beginning of treatment, rhu-GM-CSF (125µg/M²) was randomly assigned to 16 patients and injected subcutaneously twice weekly for four weeks; the other 15 patients...
